The Panel has today published its third Annual Report, which captures the learning from all the serious incidents and reviews considered in 2021.
The Panel's oversight of national and local reviews enables us to provide insight and analysis of patterns of safeguarding. They have drawn on this to highlight six practice themes which they believe can make a difference to reducing serious harm and preventing child deaths caused by abuse or neglect.
