3rd March - Learning from serious incidents

This important event for front line staff in partner agencies provided an opportunity to share and discuss the learning from local and national Domestic Homicide Reviews (DHRs), children's Serious Case Reviews (SCRs), and Safeguarding Adult Reviews (SARs).

This important event was for front line staff in all partner agencies that work with children, young people and adults as an opportunity to share and discuss the learning from local and national Domestic Homicide Reviews (DHRs), children's Serious Case Reviews (SCRs), and Safeguarding Adult Reviews (SARs). In the case of adults, this particularly relates to those those adults living with domestic violence, adults with care and support needs - such as help with everyday personal tasks like washing, eating, dressing and toileting; or help to make decisions and look after themselves.

The event was planned jointly by Safer Wolverhampton Partnership, Wolverhampton Safeguarding Adutls Board, Wolverhampton Safeguarding Children Board as an opportunity to share and discuss the learning from local and national Domestic Homicide Reviews (DHRs), children's Serious Case Reviews (SCRs), and Safeguarding Adult Reviews (SARs) into tragic cases where a child, young person or adult has:

  • been seriously harmed or died as a result of abuse, neglect or domestic violence and
  • where organisations could have worked together better to protect those involved

Around 200 staff from partner agencies across the Safeguarding Adults Board and Safeguarding Childrens Board and Safer Wolverhampton Partnership came together to share learning from local and national reviews in order to improve frontline practice and reduce the likelihood of similar occurences in future.

A superb aide memoir was also produced and disseminated at the event. It consists of brightly coloured glossy cards held together on a key ring providing a helpful prompt to frontline staff to keep the learning at the forefront of their minds when carrying out their everyday roles.
